Requesting the Cost of Your Visit

Patients have the right to an estimate of their charges. Requests must be made three (3) days in advance of an appointment. For more information click here.

Patients’ charges may vary based on the patients’ ability to qualify for the Sliding Fee Scale.

The Good Faith Estimate does not require the private pay patient to obtain other services from BHA.

There may be additional items or services recommended as part of the treatment that may be scheduled separately and are not reflected in the Good Faith Estimate.
